The quest "For Whom the Bell Tolls" begins right after you complete "Wedding Crashers." Now imprisoned in Trosky, Hans faces execution once the bell tolls 12 times. Fortunately, Henry is assigned to work as a laborer around the castle instead of remaining in prison, giving him a narrow window to rescue Hans.

Your first task is to haul sacks to a shed. This is a mandatory objective. Take the sack from the wagon and carry it to the shed beside the fenced area. Drop the sack inside, and repeat until the task is complete.

Next, assist the blacksmith. Although he's not thrilled about it, he instructs you to forge a horseshoe. Enter the forge area through the archway opposite the blacksmith. Gather the necessary materials and craft the horseshoe. Once approved by the blacksmith, head down the path you used for hauling sacks to the stables and deliver the horseshoe.

After delivering the horseshoe, it's time to help Hans. Return to the forge and retrieve the lockpick from the red pot on the shelves. Then, talk to the woman nearby, Fanka, who works as a cook in the noble's kitchen. Offering to help her grants you access to the kitchen. Follow Fanka to the kitchen; while you don't need to lockpick for her, doing so can boost your reputation and secure future food access.

Upon entering the kitchen, you'll notice the chamberlain in pain. Engage him in conversation and try to persuade him to let you brew a Digestive Potion. Success grants you access to the surgeon's workshop. If unsuccessful, you'll need to sneak in.

If you couldn't convince the chamberlain, grab some food from the kitchen to distract the guard near the non-noble's kitchen across from the forge. Otherwise, the chamberlain will allow you entry. Climb the stairs to the kitchen, find the guard, and give him the food to make him leave. Then, head upstairs to the surgeon's workshop.
Navigate the stairs until you reach a hall with two doors. Enter the right door to access the surgeon's workshop. Here, craft the Fever Tonicum and, if you convinced the chamberlain, a Digestive Potion as well. All required ingredients are in the room's chest.

If assisting the chamberlain, return to him with the Digestive Potion to gain access to Thomas' room. If sneaking, take a different route to finish the quest.
Return to the noble's kitchen and ascend the stairs to avoid more guards. Stay crouched as you move up, avoiding detection. You'll encounter a guard at the base of a staircase leading to Thomas' room. Distract him with a pebble to sneak past.
Continue up until you reach Thomas' room, where his sister, Adele, is caring for him. Hand over the Fever Tonicum to Adele, triggering the final cutscene of the quest.
